Clinical Fees

  • Intake Psychotherapy Appointment: $225 (60 minutes)
  • Individual Psychotherapy Session: $206 (50 minutes)
  • Family Psychotherapy Session: $250 (60 minutes)
  • Psychological Assessment: Fees are based on the nature of referral, please contact Dr. Luizza to discuss your needs.

Insurance

  • Insurance Reimbursement: Dr. Luizza is considered an out-of-network provider, which means that insurance companies often reimburse clients directly for her services for those with out-of-network benefits. Out-of-network means that Dr. Luizza does not have a contract with your health insurance plan provider. Upon request, Dr. Luizza can provide a monthly superbill with all of the necessary information for you to submit for insurance reimbursement. For more information regarding your benefits, please call your insurance provider about your specific plan’s reimbursement coverage. Please note that out-of-network insurance reimbursement can only be applied to psychotherapy services. Testing and evaluations are not covered by insurance.

Good Faith Estimate Notice

  • Good Faith Estimate Notice: Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy and other psychological services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare service, including psychotherapy and psychological services, medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule such an item or service. If you receive a bill that is at least $400.00 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
